A medical note can also be referred as doctor’s note. It is a written statement that is written by the doctor or other medically qualified healthcare provider to attest to the result of a medical examination of a patient. It serves as a sick note that show that a person is unfit to attend to some activities such as work, school, gym or travel among other activities.
Writing this note may not be all that easy because there is a format that one is supposed to follow to make it reasonable and accepted by the recipient. One of the best ways to make writing of this document easy, simple and clearer is to use a sample note.
When do I need a medical note
A medical note may be needed in many instances. For an employee, you will need this note when you are absent from work for more than seven days because of a medical condition. You may also need this note when you are not supposed to handle some tasks at work due to a health condition, or even when you need to be away from work for certain days of the week or month to go for medical checkups.
A school also needs the note when the student has to be away from school for some days due to medical condition or when the student needs a special attention at school still because of a health condition.
You will also need a medical note when you are not supposed to travel or work out.
How to get a medical note
A medical note is obtained from your doctor or physician. The procedure is
- The doctor conducts some tests or examinations
- The results come out and shows that you cannot handle some tasks
- The note is given with some instructions about the way forward like bed rest, working part time, change of working hours for example from night shift today shifts only and change of duties among others.
This note is specifically meant to give advice to you or the employer on how to handle oneself towards getting better. It is a note that will help your employer or the recipient to understand why working or performing some activities is not right with a certain health condition.
How much does it cost to get a medical note
In most cases one is never charged to get a medical note because it is a report of the examination that the doctor performed on you and instructions on how one is supposed to handle the daily activities.
The only time that a person may be asked to pay for the note is when you need it as an excuse to attend to other issues. For example, when you are not really sick, and you want your employer to give you some time off and the only way to get this off is by getting a doctor’s note.
